The death record index includes only the name of deceased and date of death. The purpose of the index is to let people know the record has been recorded with the state and is on file with the State Dept. of Health. These may be ordered and if anyone needs the address etc. let me know. Not all death are included, as apparently, it was not law at the time, to register deaths.
