For those questions you may not be able to answer, just put unknown. I have the Arkansas Death Record Index books for the years 1914 - 1948 in my home. These books list the names and death dates of all who died in the state of Arkansas for the years 1914 - 1948, who have a death certificate on file in Little Rock. Please realize that there were many deaths which occured in those years, especailly the earlier years, that the death did not got reported to the state, hence there will be no death certificate available. I would be happy to check the index and see if the name of the person for which you are wanting to order a death certificate is listed or not.
