Pat & List, One of at least two references to the Physician/Yankee Spy is given by Wm L McDonald in his book "Civil War Tales of the Tennessee Valley", pg 18, to wit: "A few army surgeons came with the hospital cortege that arrived in February 1862 [CSA hospital, following the fall of Forts Henry and Donnelson; later used as Union hospital]. They were assisted in caring for the patients by a number of local physicians. One of these local doctors became a Union spy. His activities were kept secret until dispatches signed by him were discovered in the official records of the U.S. Army after the war." If no one on the list knows the answer, perhaps someone will be kind enough to phone Mr. McDonald for the answer (I've talked to him by phone but don't know him well; he has a copy of my first volume and has expressed pleasure/support for the book & series).
