I have my parents Jefferson Co., AL Marriage License Record from 1940, it contains for both parties: Full name; place of residence, city/ST.; Color; Age/DOB; place of birth; occupation; full name of father; full maiden name of mother; number of proposed marriage; is applicant divorce/when/where; is there any blood relationship between parties; signature of applicant; date. Physician's Certificate (free from all venereal diseases) Marriage Bond ($200; signed by groom and another party) Marriage License (authorization to perform marriage) Marriage Certificate (date of marriage/location/authorized signature) Hope this is of help.
