Surname in CAPS, then the given name. 2. The county and state they moved from and the time. Approx. time is ok. 3. The county and state they moved to, and the time. Approx. time is ok. 4. If you wish to add a VERY short message such as in the example's below I would prefer one maybe 2 lines but no more unless the info is very good.
