My original post was 3 1/2 years ago. I do still have the file. It had gotten shuffled around and disorganized, so I finally used a binding machine to put all the pages together. I doubt they are in the original order, but it really doesn't matter. If you would be willing to pay for the postage, I'm guessing somewhere around five dollars, it's yours.
