Hi Tina, I didn't have a form when I sent for my father's SS card copy, just wrote all the info I could and they sent me the copy. I didn't have the number so I wrote his full name last address, parents and wife's names and that's about it. You could call your local SS office and maybe you could get a form from them or they could tell you how to get one if you prefer. Charlotte A.
